Practice Transformation Institute is offering training designed to educate interprofessionals with a knowledge base and skill set to enable high quality chronic disease care for patients in the ambulatory care setting.
 
Some of the learning objectives for the onsite training include:

  • Describe effective communication strategies
  • Identify approaches that impact team building
  • Identify the MQIC guidelines for asthma, weight management, and depression
  • Apply the evidence based guidelines in creating self-management support with chronically ill patients
  • Describe strategies for measuring patient self-management goal achievement
  • Recognize what Motivational Interviewing (MI) is and what makes it unique
  • Illustrate care coordination and care management activities in a patient centered medical home
  • Describe how a health coach can help a patient succeed
  • Identify the competencies when engaging in care coordination and transition of care management activities
  • Describe how a physician’s organization can help the care management and coordination team

 
Case studies and discussions will integrate the on-demand sessions with the onsite training.
 
Expectations: 100% attendance at the onsite training and a 75% or greater score on the learner’s assessment test given online after the onsite training are required to receive CE credit.

 

  
Continuing Education:
 
NURSES.This nursing continuing professional development activity was approved by Montana Nurses Association, an accredited approver with distinction by the American Nurses Credentialing Center’s Commission on Accreditation.Contact hour awarded 12.5.

  
DIETITIANS. Practice Transformation Institute is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR). CDR Credentialed Practitioners will receive 12.5 Continuing Professional Education Units (CPEUs) for completion of these activities/materials.
 

 
SOCIAL WORKERS. This course is approved by the Michigan Social Work Continuing Education Collaborative for 12.5 CE Hours. The Collaborative is the approving body of the Michigan Board of Social Work.
 

 
IACET. IACET CEUs are recognized by a wide range of organizations, including professional associations, regulatory boards, corporations and universities. Check with you organization to see if they qualify for you. Practice Transformation Institute is authorized by IACET to offer 1.25 CEUs for this program.
