PATH (Personal Action Toward Health) is Michigan’s name for the Stanford Chronic Disease Self-Management Program. It was developed and tested by Stanford University to help people learn techniques and strategies for the day-to-day management of chronic or long-term health conditions.

 

PATH Master Training

Chronic Disease Self-Management Program (CDSMP)
PTI has T-Trainers who can deliver Master Training to organizations who contract with us. Once trained, the new Master Trainers can provide Leader Training not only to others in their organization but organizations who connect with them for services. Having Master Trainers helps sustain the program in your organization and the organizations with whom you associate. Please allow three months for planning a Master Training.
 
The Chronic Disease Self-Management Program is the core program. Cross-training for the Diabetes Self-Management Program or the Chronic Pain Self-Management Program can be done by adding additional time to the Master Training or Leader Training schedule.

PATH Leader Training

Chronic Disease Self-Management Program (CDSMP)
This 4-day training will certify you to become a Program Leader for the Chronic Disease Self-Management Program. You will learn how to facilitate the program and train others to manage symptoms, communicate with health care professionals, problem solve, set weekly action plans, learn about healthy eating, and how to deal with fear, pain, fatigue, frustration and depression.
 
Diabetes Self-Management Program (DSMP)
The 4-day Diabetes Self-Management Program training will certify you as a Program Leader. It goes over much of the same information as the Chronic Disease Self-Management Program. Additional topics include avoiding the complications of diabetes, preventing low blood sugar, strategies for sick days, meal planning and foot care.
 
Chronic Pain Self-Management Program (CPSMP)
Another 4-day program in which we train Leaders is the Chronic Pain Self-Management Program. Some of the same information as the Chronic Disease Self-Management Program is taught along with other topics. Some of these additional topics include appropriate use of medications, techniques to deal with problems such as frustration, fatigue and poor sleep, communication with family, friends and health professionals, and pacing activity and rest.

PATH Workshops

Chronic Disease Self-Management Program (CDSMP)
Workshops are taught by two certified Program Leaders over 2 1/2 hours each week for 6 weeks. The content focuses on problems common to people who have a variety of health conditions, with the goal of helping participants become better self-managers.
 
The participant will learn how to create their own personal action plans and set achievable goals. Participants also learn strategies to help them manage symptoms, communicate with health care professionals, problem solve, learn about healthy eating, and how to deal with fear, pain, fatigue, frustration and depression.
 
Diabetes Self-Management Program (DSMP)
The Diabetes Self-Management Program is much like the above workshop, but with added information about avoiding the complications of diabetes, preventing low blood sugar, strategies for sick days, meal planning and foot care.
 
Chronic Pain Self-Management Program (CPSMP)
The Chronic Pain Self-Management Program is also similar to the Chronic Disease Self-Management Program. Added topics include appropriate use of medications, techniques to deal with problems such as frustration, fatigue and poor sleep, communication with family, friends and health professionals, and pacing activity and rest.
