Patient Centered Medical Home Facilitation
PTI provides site-specific assessment and targeted facilitation for practices who are striving to achieve medical home recognition through Blue Cross Blue Shield of Michigan (BCBSM), NCQA and/or URAC. We focus on identification of the specific gaps that the practice must improve to meet the recognition standards or requirements.
We will provide training for your patient centered medical home facilitators or we will work with your practice to develop local on-site support directed at achieving recognition. PTI also has the capability to provide telephonic facilitation services.
Our facilitation services include planning, implementation and toolkits for success, time sensitive action plans for closing gaps in requirements, monthly progress reports, and access to subject matter experts who can provide advice on specific challenges.
It is strongly recommended that patient centered medical home facilitation services are supplemented with appropriate PTI Learning Programs and Assessment Services.
Practice Transformation Assessment Services
If you don’t know where you’re starting from, it’s very difficult to create a roadmap for change. PTI offers a number of different assessments to support you in achieving your transformation. We will provide expertise to assist you in conducting these assessments at your practice or across a group of practice teams.

Practice Retreats
Practice Retreats are offered at the PTI training facility in Macomb, Michigan or at a site of your choice. Retreats can be designed for physicians, individual practice teams with 10 or more participants, or large groups of practice teams. Tailored training and more in-depth subject matter related to practice transformation will be developed upon request. Retreats can be designed for half day to two day sessions and more than one topic can be covered at a given retreat.

Learning Collaboratives
PTI will provide facilitation services and expert faculty to assist you in conducting Learning Collaboratives on a variety of topics. Learning Collaboratives are typically 12-month programs designed for organizations committed to achieving sustainable change within a specific topic area. Through shared learning, practice teams or teams from a variety of organizations work with each other and PTI faculty to rapidly test and implement changes that lead to lasting improvement. We have adapted our curriculum using the Institute for Healthcare Improvement learning collaborative model. Learning Collaboratives can be customized to meet different environments and learning needs. Examples of Learning Collaboratives might be on topics such as Driving and Sustaining Practice Transformation, Diabetes Management, Asthma Management, Obesity Management, Redesigning the Primary Care Office Practice, or Improving Transitions of Care.

Residency Training & Education Programs
We are currently working with family practice residency training programs in Michigan to develop training and education programs that teach new physicians about PCMH concepts and the power of quality improvement and sustained practice transformation. Practice Transformation Coaching Services & Coach Training
PTI is in the process of developing a comprehensive program for practice coach training. At this time we can provide assessment and training for developing your practice coaching program. Our experience is based on training and deploying coaches to support primary care practices participating in learning collaboratives and PCMH initiatives. The first segment of the Practice Coach Professional Development Training Program will take place in September 2010 - August 2011. Keynote Speakers and Seminars
PTI can provide subject matter experts to deliver dynamic, interactive presentations for workshops and seminars on practice transformation topics. Speakers will deliver "real life" case studies or facilitated group exercises to enhance learning and meet measurable objectives. PTI speakers leave participants with valuable tools and resources to support further learning and implementation of the learning concepts post-seminar.

E-Prescribing Training
(PTI) provides e-prescribing training and education programs online and in a classroom setting to educate providers and clinical staff on e-prescribing technologies and how to effectively utilize their key components. The ultimate goal is to encourage proper, widespread adoption and utilization of e-prescribing. PTI shares with providers and clinical staff the limitations of e-prescribing technology and how to deal with and overcome the potential of temporary productivity decreases resulting from e-prescribing workflow redesign. PTI e-prescribing training also trains participants on how to access high quality technical support and mitigate workflow change when e-prescribing systems are introduced.
