Background
The Medical Practice Coaches Network (MPCN) is unique to Michigan and is designed to serve as a contact point and an organizational learning facilitator for coaches of medical practice teams. It is the outcome of a brainstorming session between leadership of Integrated Health Partners (IHP) of Battle Creek and Practice Transformation Institute (PTI).
The two groups saw a need for a coaching component aligned with the Learning Collaborative IHP operates in Battle Creek, allowing medical practices to receive ongoing support through Learning Collaborative coaches.
Outlook for 2008-9
The MPCN is in admittedly in its early stages of development. The preliminary learning component is delivered only at Learning Collaborative events and through a regular monthly conference call schedule. One of the goals of MPCN is to deliver a practical working model of coaching for use with medical practices. The learning sessions for coaches are designed to achieve three objectives:
1. Provide field-based learning
2. Build a coaching model based on the Michigan coaches experience
3. Align learning activities with the needs of learners
The coaches’ program is a work in progress and will be developed over the coming Learning Collaborative cycle from a culture of feedback, shared learning and evidence-based practice. In 2008-9 all coaches participating in the MPCN are automatically recognized as members because of their participation in the Learning Collaborative.
Purpose
The purpose of the MPCN is to provide learning support to coaches of medical practice teams implementing aspects of the Chronic Care Model and the Patient Centered Medical Home so they can guide team performance to achieve expected outcomes.
