Is your practice a candidate for a Learning Collaborative?
Here’s a quick checklist to help determine if your practice is a candidate for participating in a Learning Collaborative:
- You want to learn how to maximize the skills and talents of your medical practice team to improve how your team delivers patient care and you are willing to invest in your team and engage them in the collaborative as appropriate
- Your practice is primary care and you want to provide more effective, better coordinated care to your patients and demonstrate your value to purchasers
- Your practice has not yet adopted a population-based registry, but is eager to do so because you understand the benefits of technology in improving patient care delivery
- Your practice has adopted a population-based registry; however, you want to learn how to use it to further maximize how you provide care management and report quality improvement measures
- You want to learn more from subject matter experts on topics such as chronic care management, transitions of care, obesity management, pediatric asthma, vulnerable elderly care, end of life care, care coordination, decision support, motivational interviewing, establishing group visits, and improving patient satisfaction
- You are a lifelong learner not only in your medical practice area but on topics that focus on continuous improvement and proven business and leadership strategies
- You want the benefit of sharing ideas and best practices with colleagues in a secure, confidential environment
- You are willing to commit to a 12 month effort to explore the learning opportunities and practice building opportunities that a Learning Collaborative affords
